Email Subject Line: Urgent: Final Working Notice

Dear (Employee’s name),

I hope this email finds you well.

As we recently discussed, your [absenteeism] is having a negative impact on both your individual work performance and the productivity of your team. We take [workplace attendance] seriously, and we expect all of our employees to [follow our company’s time-off policy].

Despite reprimands, you have repeatedly violated [this policy] and [have exceeded the number of days you are permitted to be absent from work]. More specifically, [you failed to show up at work on X, Y, Z without notifying HR or your manager in the previous month.] We recognize that unexpected issues arise from time to time, and we [are flexible (for example, by allowing employees to work from home or take a sick leave to care for themselves or a member of their family). However, we still expect our employees to be professional and act in the best interests of the company.

This is your [first] notice of caution. We expect you to [request time off in advance and in a formal manner (e.g., via email to your manager or through our HRIS.)] to correct your behavior. We will have to take further disciplinary action, up to and including termination, if this does not happen.
